P.2.7 Severe bacillary dysentery

  • Patient presented with blood and mucous diarrhoea of 3 days duration.
  • He is a known alcoholic.
  • At autopsy the colonic lumen contained a thick dark green offensive fluid - pus, faeces and blood.

 

  • Mucosa is grossly oedematous and congested, the changes being more marked in the distal portion.
  • Areas of superficial ulceration, with oval margins and sloughing floors were seen, over the whole mucosa.
  • Patchy areas of distal portion of colon showed dark green to black mucosa - impending gangrene.
