J.1.2 Perforated peptic ulcer

  • A 75 years old women presented with a history of abdominal pain, distension and backache for 2 days.
  • She died a few minutes after the admission to the hospital.

 

  • The specimen includes body and pylorus of stomach and proximal duodenum.
  • The outer surface of the stomach appears normal except for an opening near the lesser curvature in the pyloric part (shown by a probe).
  • The inner surface shows the normal rugae of the stomach mucosa.
  • The pyloric region shows bile staining and a well demarcated round, smooth edged perforation about 1cm to the lesser curvature.
  • Adjacent to it is an ulcer with raised margins.
